Attaching a photo

Every time I go to attach a photo I click the paper clip icon and I get a drop down box with an icon that reads undefined. I used to get a window alowing me to upload a photo. What am I doing wrong.

that happened to me the other night also, but my photo's still came up. So, I'm not complaining, just glad it worked.
